Summer Reading Registration Forms
Ready to join the fun? Summer Reading registration is now open! There are three forms—one for each age group—so just choose the one that fits best. You can pick them up at the Millersville Public Library or print them at home and bring them in completed to register. Fill out the form to get your reading logs and start earning prizes—we can’t wait to see you and kick off a great summer of reading!

- Each week, the library will have a new reading log that goes with our theme of the week.
- Reading logs serve as a way to track your reading progress.
- Everyone is encouraged to read throughout the week.
- ALL forms of reading are encouraged, including books, audiobooks, graphic novels, and more.
- Even having someone else read to you counts towards your reading minutes!
- Once you have completed your reading log, return it to the library.
- When readers return a completed reading log, they can pick out a prize!
